A Collaboration Between Will Grundy Medical Clinic and Will County Community Health Center

What Is This Program?

WGMC has restructured its care model to enable the clinic and its partners to better meet the health and wellness needs of our community.  The goal of this partnership with Will County Community Health Center is to provide its participants with a more holistic and comprehensive healthcare experience.  All participants will receive a triad of healthcare services that include access to wellness and primary care, access to ancillary care, and access to medications.  

What Entities Will Patients Receive Care Services From?

WGMC - Will Grundy Medical Clinic

WGMC will serve as the care coordination center for these patients. We will provide care coordination throughout all points of care to ensure there are zero barriers in securing specialist care, primary care, diagnostic tests and lab services, emergent care, and medication.

FQHCs - Federally Qualified Health Centers

In our program Will County Community Health Center is the FQHC that we work closely with. This FQHC serves as the patient's medical home, providing primary healthcare services.

Local Hospital and Health Systems

WGMC has three partner hospitals, AMITA Health Saint Joseph's Medical Center, Silver Cross Hospital, and Morris Hospital and Healthcare Centers. These hospitals will serve to treat emergent care, labs and imaging, specialty, and surgical needs of the patient.

Physician Groups and Offices

WGMC has a number of specialty care providers that we refer to. These providers will serve as the patient's specialty care providers, as well as additional primary care where applicable.

Other Health and Social Service Organizations

WGMC has various supporting organizations that, when needed, we can refer patients to. These are services that fall outside of the realm of healthcare needs. These organizations may serve as enrollment partners.
